Notice to Rental Customers
Please read carefully and sign below
THE FOLLOWING TERMS ARE PART OF THE ATTACHED
CONTRACT, NO.: 01-____________, WHICH MANAGEMENT EMPHASIZES, HEREIN,
IN ORDER TO BETTER PROTECT ITS CUSTOMERS FROM POTENTIAL LOSSES DUE
TO ACCIDENT, MISUSE, DAMAGE, THEFT, ABUSE, NEGLECT, MISAPPLICATION
AND IMPROPER CARE OF THE EQUIPMENT WHILE IN THEIR PROCESSION.
RISK OF LOSS: Any loss, damage theft or
destruction of any tool or equipment listed on the rental contract
stated above, shall be borne by Lessee (Renter), including, without
limitation, damage caused by freezing. No such loss, damage, theft,
or destruction of listed tools or equipment, in whole or in part
shall impair the obligations of the Lessee under the rental
contract, all of which shall continue in full force and effect.
Lessee will be billed for the cost of replacing an equipment or tool
destroyed beyond reasonable repair with a new unit, or if
repairable, Lessee will be billed for the replacement of damaged
parts with new ones, plus labor, if any, at $66 per hour. ALL
EQUIPMENT SHALL BE DRAINED BY LESSEE ON COLD WEATHER TO AVOID
FREEZING. LESSEE, SHALL ALWAYS CLEAN EQUIPMENT BEFORE ITS RETURN, IN
ORDER TO AVOID APPLICABLE CLEANING CHARGES. LESSEE IS RESPONSIBLE TO
CARRY ADEQUATE INSURANCE WITH PROPER COVERAGE FOR ADDED PROTECTION
TO SHIELD FROM POSSIBLE FINANCIAL LOSSES.

NOTICE
Regulations of the U.S. Occupational Health and Safety
Administration require persons who operate powered industrial trucks
to be trained and certified in accordance with the regulations. A
"Powered Industrial Truck" is defined as "any mobile, power propelled
truck used to carry, push, lift. stack or tier materials, whether
ridden or controlled by a walking operator. this included forklifts,
reach trucks, motorized hand trucks, order pickers, pallet trucks,
and the like. It also includes the Item You are renting from Us and
we also require that same training and certification as the OSHA
regulations for anyone who operates Our equipment, namely, the
Rented Item.
AGREEMENT
Accordingly, the undersigned represents and agrees that he has
read and understands the above Notice and, at all times throughout
the rental period, the "powered industrial Truck" (Rented Item) will
be operated solely by trained and certified persons in accordance
with the OSHA regulations.
