C.O.D. Customers  
To rent you must present:

• Valid ID (un-expired )
• Valid credit card (un-expired)
  (Name on the ID and credit card must match the name of renter who will sign the rental contract)  
  Acceptable ID’s are:  Driver License, Alien Card, US Passport,  
• Valid credit cart authorization form
• Proof of residence address
• Certificate of Liability and Theft Insurance (on some equipment) or obtain equipment protection insurance offered by GT Rentals
• Certificate of operator (on some equipment) 

• Certificate of Liability and Theft Insurance (on some equipment) or obtain equipment protection insurance offered by GT Rentals
• Certificate of operator or signed Addendum to Rental Contract (on some equipment)
